At a Family Doctor appointment, a Practice Facilitator may see you to:

• Gather information on your health status
• Take weight, measurements and blood pressure
• Ensure your routine screening such as blood tests or a mammogram are up to date.
• Discuss Red Deer PCN and community programs that can help you be healthy.

Patients may receive a phone call from a Practice Facilitator or Patient Medical Home Assistant regarding preventative screening tests that may be due.
