MULTISPECIALITY INTERPROFESSIONAL TEAM (MINT) MEMORY CLINIC

Who should attend?
For people having memory difficulties, living with dementia and for those who care for them. We are committed to helping people living with memory difficulties maintain independent living and have the best quality of life for as long as possible.

Program Format:
Individual appointment with care giver (2+ hrs) with the MINT Memory Clinic team.

A skilled team of professionals work with your family doctor, community agencies like the Alzheimer’s Society, and specialists to provide compassionate, person centered care. Our team includes a family doctor, registered nurse, social worker, psychologist and pharmacist. 

How will you access this service?
You will need a referral from your RDPCN family doctor.

If you are already seeing a RDPCN nurse, psychologist, mental health counsellor, recreation therapist or pharmacist you can ask them about a referral.
