Diabetes Management
Program Format:
Adults (18+)
Individuals will have a 75-minute appointment with a Certified Diabetes Educator at the Red Deer PCN Primary Clinic
Who should attend?
If you have questions or concerns about self management of type 2 diabetes and have seen your primary care provider or primary care registered nurse in your medical home, you may benefit from meeting one of our Certified Diabetes Educators. Together with your primary care provider and the certified diabetes educator can help you develop a personalized plan to manage your diabetes with insulin or other medications, and make recommendations for changes in your lifestyle and eating habits.
How do you access this service?
- You can be referred by your RDPCN primary care provider or,
- If your primary care provider is part of the RDPCN you can self-refer by calling 403.314.3297.
- If you are already seeing a RDPCN Nurse, Psychologist, Mental Health Counsellor, Recreation Therapist or Pharmacist you can ask them about a referral.
Your commitment:
Bring a list of your questions and concerns and be prepared to work together on a plan for positive change. If you have a device to test your blood sugars and a record of blood sugars, it would be helpful to bring those with you.
