  • You can be referred by your RDPCN primary care provider or,
  • If your primary care provider is part of the RDPCN you can self-refer by calling 403.314.3297.
  • If you are already seeing a RDPCN nurse, psychologist, mental health counsellor, recreation therapist or pharmacist you can ask them about a referral.

Maternity Care
If you do not have a primary care provider or if your primary care provider does not deliver babies you will need to have the pregnancy confirmed either at a medical (clinic) home or by your primary care provider.

The primary care provider will assess whether your pregnancy is high or low risk. High risk pregnancies will be referred to an obstetrician.

For a low risk pregnancy, your primary care provider will submit a RDPCN Low Risk Maternity Care Referral Form to the RDPCN.  The RDPCN will forward the referral to a clinic accepting low risk maternity patients.  The clinic will then contact you to set up an appointment.

Your commitment:
Bring your list of questions and concerns for discussion.
