MULTISPECIALITY INTERPROFESSIONAL TEAM (MINT) MEMORY CLINIC

Program Format:
Adults (18+)
Individuals with a care support person will have a 3 hour appointment with the MINT Memory Clinic Team.
A skilled team of professionals works closely with your primary care provider, community organizations like the Alzheimer’s Society, and specialists to provide compassionate, person-centered care.
Our primary clinic team includes a Primary Care Provider, Nurse Practitioner, Registered Nurse, Social Worker, Psychologist, Pharmacist, and the Alzheimer’s Society.

Who should attend?
This program is for anyone experiencing memory difficulties, living with dementia, or caring for someone who is. We are committed to helping people with memory challenges live as independently as possible and enjoy the best quality of life for as long as possible.

How will you access this service?

• Referral from your RDPCN primary care provider or,
• If you are already seeing a RDPCN nurse, psychologist, mental health counsellor, recreation therapist or pharmacist you can ask them about a referral.
