By completing the survey at your first &/or midterm prenatal appointment.
If your primary care provider is part of the RDPCN you can self-refer by calling 403.314.3297.

Maternity Care
If you do not have a primary care provider or your primary care provider does not deliver babies you will need to have the pregnancy confirmed either at a physician clinic or by your primary care provider.

The primary care provider will determine if your pregnancy is high or low risk. (high risk will be referred to an obstetrician)
For a Low Risk Pregnancy a RDPCN Low Risk Maternity Care Referral Form will be submitted by the primary care provider’s office, and forwarded by the RDPCN scheduler to a clinic that is accepting Low Risk Maternity Care Patients. You will then be contacted by the clinic accepting your care to set up an appointment.

Your commitment:
Bring your list of questions and concerns for discussion.
