Memory Testing
Program Format:
Adults (18+)
Individuals will have a 75-minute appointment with a Primary Care Registered Nurse.
Who should attend?
For anyone noticing changes in memory, thinking, perception or the ability to carry out daily tasks. Your primary care provider will review the results with you.
How do you get into the program?
- You can be referred by your RDPCN primary care provider or,
- If your primary care provider is part of the RDPCN you can self-refer by calling 403.314.3297.
- If you are already seeing a RDPCN nurse, psychologist, mental health counsellor, recreation therapist or pharmacist you can ask them about a referral.
Your commitment:
- Complete one or two brief written tests. These screenings help identify any changes in memory or thinking.
- Write down any questions or concerns and bring them to your appointment.
- It is recommended to bring a care partner or family member for support.
Where is the Program located?
The Primary Care RN is located within your medical (clinic) home.
